Reviewing Activities, Goals And Objectives

A review of an outcome plan may lead to new activities, goals or objectives being added or existing activities, goals or objectives being reviewed and/or completed depending on the clients circumstances. To identify that these changes were made as a result of a review, the addition, review and completion of these elements of an outcome plan are automatically associated with the review that is open at the time the action was performed by a case worker. For example, if a review created on an outcome plan on 1 January 2010 and a service was added by a case worker to this outcome plan on 2 January 2010, the addition of this service is automatically associated with this review. As a result, the case worker can identify that the service was added as a result of this review. To allow a case worker to easily identify added, completed and reviewed activities, goals and objectives, a list of added, completed and reviewed elements are visible from within the review. To allow a case worker to easily identify elements which have not yet been reviewed, a list of activities, goals and objectives which have not yet been reviewed is provided.

The case worker will also be provided with a list actions that have been performed on activities, goals and objectives which will allow them to identify the actions performed on each element as a result of a particular review. Note: This is not provided for referrals as it is envisaged that referrals are not reviewed or completed as they are not paid for or tracked by the agency. The addition of referrals will be visible from within a review if they were added while a review was open.

When the case worker has reviewed all necessary aspects of the plan the review can be completed.
