Service Plan Template

When a service plan is based on a template, a case worker will decide on the appropriate service plan template following an assessment of the needs of the plan participant. Once the case worker has chosen the appropriate template, the service plan is created. The service plan will use the service plan elements configured as part of the administration of the template within the administration component.

The expected duration of each plan item is pre-configured during the administration of the service plan template, hence each plan item will be created with pre-defined expected start and end dates and an expected outcome. Authorized units will also be pre-defined if a service unit delivery plan item has been added to the template. The expected duration of each milestone is also pre-configured during administration, hence each milestone will be created with predefined start and end dates. Any approval criteria pre-configured during the administration of the service plan template will be associated with the respective plan item.The case worker can adapt the service plan to meet the specific needs of the client by adding or removing any of the predefined sub-goals, plan items, or milestone if required. If a particular plan item has been pre-configured during the administration of the service plan template to be mandatory, then the plan item may not be removed.
