An IIOP Connection that has already cleaned up by one thread is
still being used by another thread

The customer is now working successfully. The issue in trying
to correct this problem is that we don't have the needed
documentation.  Here are the original investigation findings:

In looking at the code, here are the conclusions:
.
It is possible for both register_synchronization() methods to
raise Inactive.
.
If we take them from the bottom (in Cruise index), the
OnePhaseOnlyCoordinator throws inactive if committing is true -
this is turned on in before_completion() (definitely if no
Synchronizations are registered), conditionally otherwise -
only if no exceptions thrown by any Synchronizations.

In the other case the Sync throws inactive in the add() method
(called during register_synchronization) if closed is true.
This is turned true during beforePrepare() ( here it is turned
true if last Synchronization is being called for
before_completion(), or if rollbackOnly becomes true), and
afterResolution() ( here it is turned true if it was false).

The two files mentioned in the PMR had no trace entries -
just the exceptions being reported.

The PMR mentioned appears to be one to do with problems in the
ORB - there are no files on the Level 2 site to help us with
determining the cause of this error.

Without the trace information, there appears to be no way to
see what is causing the error - we really need to see what
went on before the exceptions start.
