Unmapping the custom exceptions (e.g. DuplicateKeyException)
may not work.

* PROBLEM DESCRIPTION: If a customer wished to un-map an       *
*                      SQLException we had mapped              *
*                      for example to DuplicateKeyEx.          *
*                      It would not work and they              *
*                      would end up with our                   *
*                      DuplicateKeyEx.                         *

There was incorrect logic when processing a removed map.
Problem conclusion
The code now correctly removes a map (or does not map) an Error
Code or SQL State.
