When a waiter wakes up because it timed out, it still has
1 or 2 milliseconds left to wait.  So, it is stuck back on
the queue, and another waiter is stuck on the queue
accidentally.  These wake up before all the time is
gone again and the cycle repeats.
Problem conclusion
The fix involves adding a flag in the waiters object to be
set when the waiter is woken up on notification of a
connection being returned to the pool.  We now know for
sure when a waiter should go back on the queue to finish
waiting if it can't get a connection, or if it just timed
out and should be gotten rid of.
