Error description
After the removal of a node, the node information is not removed
from the WLMBOOTSRVR_TABLE. This fix removes the node
information from the WLMBOOTSRVR_TABLE during node removal.
Local fix
SQL scripts ran against the UDB repository table
(WLMBOOTSRVR_TABLE) to remove the phantom node entries.

After a node has been removed from the adminstrative domain
through the use of the admin console (or other supported System
Management ways), the node name still appears in the
WLMBOOTSRVRS_TABLE.  A symptom of this is the admin server
may take longer then expected to start.  This is verified
when the XMLConfig is fully exported and compared against
the node entries in the WLMBOOTSRVRS_TABLE.  The
WLMBOOTSRVRS_TABLE will contain the names of removed nodes
and the XMLConfig export will not.

Problem conclusion
The wrong SQL statement was being used to remove the node name
from the WLMBOOTSRVRS_TABLE.  The resolution was to use the
correct SQL statement.  Specifically, the key that corresponded
to the SQL statement was wrong, not the actual SQL statement
itself.
