* RECOMMENDATION:                                              *

ServletInstanceReference objects accumulated in the servant
region and were not eligible to be garbage collected.
Eventually, this resulted in the java.lang.OutOfMemoryError.
Problem conclusion
ServletInstance object has been modified so that it does not
cache ServletInstanceReference objects.  This will allow the
ServletInstanceReference objects to be eligible for garbage
collection.
