PQ88994 resolves the following problem:

When attempting to start WebSphere® Application Server, the Application Server does not startup cleanly. In such a configuration, the Application Server must be configured to use replication with Dynamic Caching where many caching instances are created in one JVM™.

A hang and an IndexOutOfBounds exception are caused by deadlocks between threads completing initialization and threads attempting to use resources that are still being initialized.
