Load balance was not equally distributed to all available
servers once a downed server is brought back online.

When using server weights, the plugin failed to route
requests to a downed server once the server was restarted.
The plugin was incorrectly updating the server connection
count resulting in requests not being routed to a previously
marked down server.
Problem conclusion
The plugin was modified to correctly update the server
connection count so that a downed server will be retried
when server weights are used.
