Closed as program error.
Error description
Whenever cluster members with duplicate server names are
migrated, one duplicate overwrites the other during migration.
In the case of two cluster members with the same server name,
the first duplicate is overwritten by the second and is no
longer in the new cluster.xml file.
Local fix
Work around is to add the missing server back to the cluster
using the admin console.

Servers with the same name and that belong to the same cluster
fail to migrate properly. The last server with the same name
will overwrite the prior server, leaving only one server per
name.
Problem conclusion
Code has been added to not only check for server name, but also
to check for the node it belongs to, thus allowing for servers
with the same name.
