Error description
WebSphere throws 500 errors when attempting to handle requests
WebSphere's plugin marks the application server as down and fail
s to handle any future requests to the application server until
the applications server is recycled. When looking at trace, onl
y  one transport thread is being allocated to receive requests a
nd  the request is never passed to the webcontainer.
Local fix Problem summary

* PROBLEM DESCRIPTION: WebSphere's pooling of transport        *
*                      threads becomes corrupted causing       *
*                      user requests to fail with http status  *
*                      codes of 500.                           *

* RECOMMENDATION:                                              *

WebSphere allocates a pool of http transport threads to
handle requests.  Due to a bug in the freeing of a thread
back to the pool, a thread could be added to the list of
free threads multiple times.  If the thread is added to
the list incorrectly, the pool could get corrupted causing
WebSphere to not properly allocate threads to handle
http requests.
Problem conclusion
Modified the pooling of threads to not add a thread to
the list more than once.
