Leak in Dynacache under the com/ibm/ws/cache/CacheServiceImpl
caused by references to disk offload objects not getting
cleaned.

When cache replication is enabled and the replication
mode is PUSH_PULL, a memory leak will occur. This
might result in a permanent out-of-memory exception.
There are no other exceptions logged.
Problem conclusion
The problem was caused by a code error in the Dynamic
Cache to DRS handling of PUSH_PULL replication entries.
The memory leak will only be seen if you are also
using the disk offload function. The code was
modified to correctly handle shared cache entries
of PUSH_PULL type when the entries also reside on the
file system.
