Introduction

To successfully complete an outcome plan and achieve positive outcomes for clients, it is important to regularly monitor and evaluate a clients progress. By regularly communicating with the client, a case worker can identify progress and also identify if the activities outlined in the current plan are sufficiently addressing client needs.
