Creating An Outcome Plan For Clients

An outcome plan can be created by a case worker in a number of ways. It can be created from an integrated case, it can be created outside of a case or it can be created automatically by the system.

When creating an outcome plan from an integrated case, all members of that integrated case are available for addition to the outcome plan. The integrated case is then automatically related to the outcome plan.

When creating an outcome plan outside of a case, the Citizen Context Viewer (CCV) is provided which allows a case worker to search for a client and to drag and drop the client, their cases and family members from the CCV into the outcome plan. In certain circumstances an agency may choose to have the system automatically create an outcome plan. For example, when an income support case is created for a client, an agency may choose to automatically create a self sufficiency outcome plan which will be used to help the client return to work and become independent of the organization.

Ownership of an outcome plan is assigned to the case worker creating the outcome plan. An agency can also specify their own default ownership strategy. An ownership assignment strategy can be defined in outcome plan administration which can be used instead of the default ownership strategy provided by Cúram. Standard Cúram supervisor assignment functionality is used to assign a supervisor to an outcome plan. Outcome Management provides the ability to view or maintain the plan owner and plan supervisor at any point in time.
